What are the supporting documents do I need to prepare before making a claim under the Policy from Chubb?

a copy the proof/screenshot from the Atome App of policy status or coverage period 

The deceased Insured Person’s:

  • Death certificate
  • Driving license (if involved in motor vehicle accident)
  • Related police report, sketch plan, photograph and investigation result by the Police authority (if involved in motor vehicle accident)
  • Detailed postmortem report
  • Toxicology report if blood and urine specimen were taken for test
  • All relevant diagnostic / pathology/ laboratory/ Imaging report
  • Burial permit

(i) Medical report and all relevant diagnostic / pathology/ laboratory/ Imaging report

(ii) Detailed Specialist Report by the treating doctor on his present condition is required to determine policy liability
